DESCRIPTION:The International Foundation of Integrated Care (IFIC) in partnership with RIVM and Vilans presents the 18th International conference on Integrated Care “Value for People and Populations: Investing in Integrated Care” to take place in The NBC Congrescentrum\, Utrecht\, Netherlands 23 – 25 May 2018. DESCRIPTION:Integration is the new buzzword. A wave of change is happening all over the world across all sites and levels of care – in the office spaces of general practitioners\, in community care providers and hospitals – to reorganize care in a more coordinated and integrated fashion. Unfortunately\, the innovations of successful reforms often remain local\, unheard and unspoken of\, with no platform for sharing and discussion. To witness the effects of integration on systems\, providers\, professionals and patients\, we are holding the Global Conference on Integrated Care again at the Resorts World Convention Centre in Singapore from 1st – 3rd February 2018. This Conference brings together health and social care experts\, practitioners\, professionals and policy makers under one roof\, to present and discuss an array of topics related to integrated care. SUMMARY:Scaling Integrated Care in Europe – What does it take?
DESCRIPTION:More tools are needed that can help us to understand how to move towards more sustainable health and care systems\, and how to support implementation\, scalability and transferability of integrated care solutions in Europe. \nThe objective of this workshop is to explore the SCIROCCO tool’s potential to facilitate exchanges of good practice and the scaling-up of active and healthy ageing solutions in Europe. The workshop will demonstrate the development\, validation and testing of the tool. It will show how the tool will build on the results of its application to 15 good practices in integrated care in five European regions. It will provide a unique opportunity to gain a more detailed insight into the parameters of the SCIROCCO online tool \nMain messages \n\nThe SCIROCCO tool can help you to scale up your integrated care/active and healthy ageing solutions.\nLooking at example good practices can help you understand integrated care in your own region.\nScaling up of integrated care requires understanding of the context in which successful practice emerges.
